Navigating the ever-growing web of senior care and housing can be overwhelming for seniors and their families. At Elderwerks, our case management staff offer free one-on-one support, referrals, and guidance to over 3,000 seniors annually. Our case managers are experienced aging professionals that guide seniors through available benefits, accompany clients as they explore senior living options, and ensure that any decisions made by seniors are aligned with their stated goals, needs, and values. As an independent non-profit, we have the ability to work with clients at their pace and with the level of support they desire to ensure they and their families are confident in their decisions.
Each year, Elderwerks provides information, training, and in-person services to over 11,000 seniors and their families. Our mission is achieved through an array of services including individualized case management, community education, continuing education for aging professionals, caregiver support groups, dementia training and services, and the publication of the 340+ page Elderwerks Senior Resource Directory that is hand-delivered to over 10,000 households throughout Illinois.
To provide older adults, seniors and families information, referrals and guidance needed to make appropriate decisions regarding senior living, home care and supportive services. This includes education on senior topics for community members and professionals. The guidance we provide helps seniors stay home well or transition successfully into a new senior living opportunity.
