Description of project

The faith community nurse works with seniors who live at independent senior facilities, affordable housing units, senior outreach ministries and senior community residents who utilize the senior box programs through food pantries. The initial and ongoing visits with the senior consists of health screenings/assessments, health education and appropriate spiritual and social service referrals. The client health education includes, but is not limited to, healthy lifestyle and disease prevention, chronic disease management, medication management (what, when, where and how to take the current medications), fall prevention and safety, identification of accessible resources, and identification of when to utilize the emergency services or primary care.

Quarterly group education classes may occur at each site location. The topics of the classes include but are not limited to, healthy lifestyle and disease prevention, chronic disease management, fall prevention and safety, spiritual resources to use for healthy lifestyle, available social service resources to assist the senior to live safely and independently in home, and appropriate exercises for balance and strengthening for older adults. The group classes range in size from eight to 12.

The nurse enters client contacts, home visits, phone calls, and group pre and post-tests in the Deaconess Faith Community Nurse Ministries on line HIPPA secure database.

The impact of the program is that falls were reduced 17% for the seniors after visiting with the nurse and implementing fall prevention strategies. Additionally, 76% of the seniors presenting with symptoms of depression improved with nurse interventions. The program aims to improve the lives and health of seniors, so that they can thrive in their current place of residence. Our goals for the program include improve self-management of prescription medications, improve chronic disease management among seniors and decrease injuries from falls among seniors.
