Our Services For Older & Vulnerable Adults

Independence with dignity…that's what Catholic Family Center's (CFC) services for older and vulnerable adults are all about. With the right supports, older adult clients are able to stay in their own homes and out of costly nursing home care; vulnerable adults are protected by legal guardianship services and/or budget & bill paying oversight. Here is a brief description of some of our supporting services for aging adults and their caregivers:

STAR (Support to Aging Residents)
•Basic needs services to help our beloved aging residents stay independent and at home.
•Grocery shopping;
•Transportation to medical appointments and other essential appointments;
•Prescription pick-up;
•Banking, chores, friendly visiting, errands, visits to loved ones;
•Home repairs & maintenance (HomeWorks)
•Services provided by a unique blend of trained volunteers and paid staff.

•Simple one-stop source for long term care and senior services information;
•Phone consultations for anyone at no charge;
•In-home assessment by geriatric care managers;
•Access and linkages to other services and programs;
•Eldercare specialist provides guidance on the spot or refers to geriatric case manager.

Expanded In-home Services for the Elderly Program (EISEP)
•Helping elders stay independent in their own homes
•In-home assessment of needs by skilled case managers who then link clients to appropriate services and monitor those services on an on-going basis;
•Subsidized housekeeping including light cleaning, laundry and meal preparation, personal care including bathing, dressing & grooming.

Financial Management (FLAGS)
•Management of vulnerable individuals' SS/SSD/SSI income through establishment of a budget and arranging for bill payments through the Representative Payee Service or when approved as trustee;
•Performance of all personal/property responsibilities of a legal guardian (as detailed by order of the court system).

Beverly, 70, an EISEP (expanded in-home services for the elderly) client since 2007: "Without EISEP, I couldn't live here (Kennedy Towers) and the other options in the city weren't right for me. EISEP is my saving grace. It is a very, very essential service."

Lakesha, caregiver for her 87 year old grandfather: "My Grandpa, who lives alone, was becoming increasingly withdrawn and seemed sad all the time. I was concerned that he might be suffering from depression but I had no idea who to turn to for help. A friend told me about Eldersource. I called them and spoke with an eldercare specialist. She connected me to a Geriatric Mental Health Specialist who ultimately came and visited Grandpa, evaluated him and helped convince him to have treatment. I believe she and Eldersource saved my Grandpa's life."

Floyd, 82, a STAR (support to aging residents) client since 2010: "My son and daughter-in-law do all they can for me but they work full time and have three kids in high school. My STAR worker is a true gem and has made it possible for me to continue living on my own and to not be a burden to my family."

Real life scenarios shared by FLAGS (financial management) staff: "I have a client who was living in a local "hotel" in very run down conditions that cost him nearly his entire month's social services resources. I found him an apartment in a safe neighborhood, helped him get furnishings and a TV, made arrangements for housekeeping, made sure he has food. Last week, I made a home visit and his apartment was spotless, his kitchen well stocked and he was watching his TV. He told me he was very thankful and grateful for all the help I gave him. He is stable and happy and no longer calling me two or three times a day with problems."
