Vive, meaning "long life" is an innovative project that is designed to nourish people 65 or better within their home and community for as long as it is safely possible. The goal of Vive is to integrate evidence based strategies and interventions into low and moderate income neighborhoods. The types of activities and resources that are provided by Vive have a positive impact on upstream determinants of health and promote inclusive aging in community systems for individuals who experience health disparities. Vive programs focus specifically on seniors who are very low income and have been identified as having extremely limited access to resources and activities that support their health and wellbeing. This innovative aging in community model blends institutional, home and community based services with formal and informal care designed to improve access for people who have become isolated or who find it challenging to participate in healthy living programs.
Vive includes health and wellness services, nutritious meals, classes and workshops, community events, and intergenerational activities within low-income senior communities. A Community Navigator meets with seniors in each neighborhood to identify areas of interest and gaps in services and resources. The Navigator then invites local agencies and volunteers into the community to provide fitness classes, health screenings, serve meals, engage with seniors, and create a vibrant community. Seniors who live in Vive enhanced neighborhoods report significantly higher levels of access to health and fitness supports and improved independence with daily living activities. In this way, Vive supports a healthy aging in community model for some of our most vulnerable seniors.
